Enrollment in LW-21 Club
How do I enroll?
If you did not receive a letter from us directly encouraging your child's participation, you can download an enrollment form on our "Forms" page. Due to the requirements of the grant, students must be recommended for the program. Once your enrollment form is received, we will have to consult with your child's teacher to determine eligibility. Our site coordinator will then be in touch with you regarding a start date for your child.
Please fill out the form in its entirety, then sign where indicated. Seal all in an envelope addressed to LearningWorks-21 Club.
Hand in to your child's teacher, to the school office, or mail to your child's school with the recipient: LearningWorks-21 Club
